To be used where it is intended to transfer an existing sex entertainment venue licence to another operator

  Application to transfer a Sex Entertainment Establishment (PDF)
  Application to transfer a Sex Entertainment Establishment (Word)

 

To be completed by the current licence holder who has agreed to the transfer

  Consent to the transfer of an existing sex entertainment establishment licence (PDF)
  Consent to the transfer of an existing sex entertainment establishment licence (Word)

 

To be completed annually by staff employed in a sex entertainment establishment
